then / now

Hi,

"She was advised by her doctor to have the surgery then / now, rather than wait until later."

Should 'then' or 'now' be used in a sentence in the past tense such as this example?

It depends whether the advice applies to a surgery that was to have taken place in the past or did take place in the past (then) or to a surgery that she is stil

The assumption on my part is that the advice was given some years back.
