THEN how do you know?

Hi.

Guy 1 - How's your dad?

Guy 2 - Fine.

Guy 1 - Have you talked to him lately?

Guy 2 - No.

Guy 1 - Then how do you know?

1. Is "then" perfectly natural in the last line of dialog?

2. Also, would it be more natural to add "how he is?" or "he's fine?" to "Then how do you know..."?
