When should one use a comma with 'then'?

Hi!

I know that a comma could be used in:

'If you do that, then he will not come'

It's in sentences such as the following that confuse me

'He went to the park then went to his friends'

'He passed his exams and then went to university'
(please ignore if duplicate)

'He went to the park, then went to his friends' ( I would put a comma after park. )

'He passed his exams and then went to university' ( I would not put a comma after exams. )

I think there is a difference between BrE and AmE in regard to the punctuation of the sentences.

He went to the park, then went to his friends'
'He passed his exams and then went to university'

He looked at her, then turned away.
He studied medicine at Camb
