The verb "does" refers to here.

This novel is set in a Chinese village before World War One.
The protagonist lives with his wife, children, and father, his wife was a maid with very rich family.
The province is suffering from a great famine.

She would give birth alone, squatting over the old tub she kept for the purpose, creeping about the room afterwords to remove the traces of what had been, hiding as animal does the birth strains of its young.

I'd like to know if "does" refers to "hide."

You're on the right track. 'does' is a pro-verb for 'hides'. (This is not "referring". There is no object in the real world that is a 'does'.)

(It was probably "as an animal does", by the way.)
