The use of Who

How is the grammar structure for a question with 'Who' ?
this is my doubt:
- Who is talking to Alex ?
- Who is Alex talking to now ?

Why does the subject changes it position ? Thanks

A question is like a statement where you have to fill in the blank.

1 Who is talking to Alex? = ___ is talking to Alex.
2 Who is Alex talking to now? = Alex is talking to ___ now.

In 1 you don't know the subject of the sentence. That's what you are asking
