...the rest remain / remains as is?

I'm a little confused with this. I'm writing a letter to a newspaper ad agent for some adjustments in our advertisement. I wanna know the correct grammar, how am I gonna write this?

My letter goes like:

Attached herewith is a copy of the information I would like you to add in our page. For some adjustments, kindly remove the last three lines in it and the rest remain/s as is.

I hope to get an answer asap. Thank you so much!
