The past simple tense

Hello!
I am wondering about the use of the past simple in the following sentences:
Are these sentences correct?
Would you like to know whether Greg commited suicide or killed one of his friends?
Did he stay in captivity or he left the camp in a different way?

Did he stay in captivity or left the camp in a different way?

Did he stay in captivity or leave the camp in a different way?

(are we still referring to 'did' here and therefore should use the infinitive or both
'left' and 'leave' and possible?)

Did he stay or leave? (Not left.)
Did they drink or eat? (Not ate.)
Did she run or walk or jump or skip? (Not walked, jumped, skipped.)
