The past perfect with "now"

hello? ^^

I found this sentence while studying English

I had now been in the United States of America something like five years,

working here and there as the inclination seized me, which I must confess, was not often.

I am wondering how can write "now" in the past perfect sentence.

in this sentence, what does "now" mean?

When can I use "now" with the past perfect?
