The meaing of a phrase "have as a referent"

So I was looking the verb "refer to" up in a dictionary.

In https://www.lexico.com/en/definition/refer it has this definition: "refer to(of a word or phrase) describe or denote; have as a referent"

I actually have 2 questions:

1. What does this phrase "(of a word or phrase)" relate to? Subject or object? For example: The telephone (subject) refers to a device (object) which allows you to make phone calls.

2. What does "have as a referent" mean? what does it relate to?
