"The last time" and a "had" question

Hi there! I had an english test few days ago and there are 2 questions i'd like to know the correct answers to.

the first one is a Rewrite question: "Rewrite the following sentence using the word in the brackets to give the same meaning", and it's as follows:
-Everytime i wash my jacket, its buttons break. (The last time...)
My answer was: The last time i washed my jacket, its buttons broke.

some of my friends wrote "had washed", others wrote "were broken", i myself was very skeptical while writing it and i'd appreciate an explanation to why it's right or wrong.

The second one was a "Choose the correct answer" question:
-Between seven and six am this morning I (had - was having - had had) breakfast.
My answer was "was having" for this one, but again, i'm skeptical even more that most of my friends chose "had had", an explanation would be welcomed here as well.

The verb "broke" is correct.
