The last questionnaire found that well over.......

"The last questionnaire found that well over 60 percent of the respondents chose over other media as their major source of information"

My grammar book says 'the last questionnaire' in this sentence is ungrammatical and I really don't know why. Please tell me why the underlined 'the last questionnaire' is not grammatical.
