The difference between take medication and be on medication

Hi Tutors~


I'd like to ask you how different take medication is from be on medication.

My guess is if someone has been taking medication constantly they say they are on some type of medication and take medication is actual action.

Ex) I took a pill yesterday.


Now I am wondering if they are interchangeable.

I don't think I understand exactly.
