"The Chaos" - poem

Hello!
I am in desperate need of PHONETIC transcription of this poem. I searched the internet high and low and couldn't find it. If anyone knows a page with it, or perhaps has a copy and could send me a scan of it, I would be much obliged.
The poem starts with words "dearest creature in creation" and is quite popular in english teaching worldwide.
