The cause of death as heart attack/ heart attack as the cause of death

Two and one-half hours after discharge from the hospital, the patient was found dead at home by his wife. The death certificate gave the cause of death as heart attack.

I'd like to know I can switch positions of both "the cause of death" and "heart attack."
Thank you in advance for your help.

'Always' is a little strong. The Corpus of Contemporary American English has 167 citations for 'from hospital', including 32 for 'discharge/discharged from hospital'.
