Philip loves the ex-waitress, Mildred, who was a German's mistress, so much. he arranged for her to meet his handsome friend Griffiths.
Afterwards, they fell for each other.
Heyward is best friend, who know nothing about Mildred.

He was glad now that Hayward was not in London on Saturday afternoon when, distraught, he went in search of human comfort: he could not have prevented himself from telling him everything, and Hayward would have been astonished at his weakness. He would despise him, and perhaps be shocked or disgusted that he could envisage the possibility of making Mildred his mistress after she had given herself to another man. What did he care if it was shocking or disgusting? He was ready for any compromise, prepared for more degrading humiliations still, if he could only gratify his desire.

1. I don't think "he" can be modified by an adjective placed before it.
So I was wondering if "distraught" means "being distraught."
2. I'd like to know if "if it was shocking or disgusting" is a concessive clause.

Ah, yes. It hadn't occurred to me that you didn't know that idiom. It's a rhetorical question, i.e., a question whose answer we already know or can easily guess. The grammar is quite irregular and thus not worth the trouble to rationalize in terms of standard grammatical
