That-deletion

I'd appreciate it if someone told me which of the following sentences are grammatically correct. (These sentences don't have contexts, so I can't include them.)

a) John, who do you think won the superbowl?
b) John, who was it that killed him?
c) Mary, who do you think that was killed?
d) James, who do you think that killed Francis?

The first two are correct. The second two are not - drop the 'that'.

Oh, and what about the sentence-

Who do you think that was killed by Mary?
