Thank you note

I wanted to write a thank you note to my Physicians assistant and was wondering if someone could help me with any suggestions? Also, I'm not sure how to sign the closing. Thanks, here is the note I drafted.

Dear ....,
I want to thank you for being my P.A. and taking me as a new patient during this past very difficult year for me. I appreciate your talent, care, patience, and time you take to listen to me when I need it. Being on the patient side in the medical world these days can be challenging, probably as it is on your side, too. But you made it easier for me with scheduling and being able to get a hold of you, etc. and I really appreciate that. And I'll never forget you were the one who took that horrible tube out of me. Thank you so much!!!
