Thank you letter to therapist

I wonder if someone could proof read this for me?
Also, how should I sign it? It's officially a 'professional' relationship, but not really, not a friend either...

I decided to write you a small note, thanking you for helping me this past while.

I never thought therapy would work for me and that talking about it would make it worse, 

but I couldn’t have been more wrong. Words really don’t describe how far I’ve come with your help. 

Thank you for helping with my problems, for listening and talking, giving me words of wisdom, 

encouragement and always being very patient.

Thank you for being a genuine, caring person as well as a great nurse, I know this is your job and all, 

but that doesn't take away from how wonderful you've been. 

Who knows where I'd be right now had you not been caring for me. And really caring. 

There really needs to be more Tonias in the world. 

I think you're an exceptional person and I think I lucked out to have you as my nurse.
