Tests (Check My Answers)

My answers are blue.

The teacher says the correct answer is 'in'? But I think, at.

It’s clear that 'in the bath'. But can there be a spider ON the bath?

Which to choose? In fact, can they all be used. Or am I missing some rule?

The second option is Past Simple Passive and it should come up more, but the answer is #1. Why?
