Tenses and verb

Which one is correct? The question has used for TOEIC exam and the answer in the book "Cambridge test" is (A). However, I don't understand why the answer isn't (B) as we have "recommended" for the first verb and the second should be "continued" with "ed".

Mr. Walton's physical therapist recommended
that he...........the treatment until the
swelling has gone down.
(A) continue
(B) continued
(C) has continued
(D) is continuing

I have to pay more attention for the verbs that associate with subjunctive form.
Is the subjunctive form use to underline the importance of something such as the advice of the doctor as in the question?
