Tenses

This multiple choice is from a test paper.

---I have failed in the driving test again. I ___ hard on it but….
----Never say die. In fact, the new rules are too harsh but you are making progress.
A. worked
B. had been working

The answer is B.
I feel the question is wrongly designed.
a) We should say “work hard at” instead of “work hard on”;
b) The answer should be A;
C) If we use B, we should change “have failed” into “failed”.
