Tense Simplification

I had never heard of tense simplification until today. Marius Hancu brought this subject up, and I was just wondering if the following is an example of tense simplification.

She hadn't been informed that he (had) disappeared.

How about this one:
She had noticed that he did [not had done] his part.

Native English speakers I've talked with on IRC don't seem to know anything about tense simplification, and they all said the above sentence is wrong. I found only 181 results when I googled for "tense simplification." Do you use tense simplification in real life? Is this a legitimate subject?

Thanks for your help.
