2) This one comes up fairly regularly. I know that many people think "Did he use to ...?" is correct, but to me it looks horrible. I am mentally unable to compute the grammar of it.
The other option, "Used he to ... ?", is structurally preferable in my opinion, but unfortunately it is too formal (or even stilted) for ordinary colloquial use.

fivejedjonI feel the same about the second most popular form, did he used to?.For me, that one is almost as horrible.
