Tense

Suppose Michael was in a car accident yesterday:

"It (was, has been) the second time he (was, has been, had been) in an accident."

What tenses should I use at the two spots?

The writer is viewing it as the 13th. time in the period of the past up to that Monday.
I wouldn't say that the Present Perfect would be wrong in the sentence, either.
