Tell me whats the difference

Hi Teacher,

The sentence below, should i apply tell or told.
either the answer is yes or no, can you please explain why in a grammatical rules.

Question 1
"she forgot to told me to participate in the competition"



What's the difference from below sentences?

Question 2
Has James been to chruch before?
Did James been to chruch before?
