"tell" in direct speech.

I wonder if it its correct to put "tell" in a direct speech like the below. Also I want to know if "tell" can be used without an indirect object like 'me'.

a. She told me "I want to go home early"

b. She told "Iwant to go home early"


The below sentece is from the book I study at the moment. and the book says it is correct. However I don't think this sentence is logical since the main verb "told" is past tence.

c. Ann told me that she is happy with my present.


I think the sentence should be changed into like the below.

d. Ann told me that she was happy with my present.
