Teacher Email

I am doing work experience at a school near me and am trying to write a letter to this teacher, however I do't know how to address this teacher, I don't know the teachers marital status and I do not want to offend her.
I would prefer to do it in either of these ways:

1) Mrs
2)Miss
3)(Full name)
4)(Full shortened name) i.e. Elizabeth would be Liz

I am open to accept other ways
