'take responsibility' or 'take on responsibility'?

I am an adult now. After growing up, I think it's time to take (/ take on) full responsibilities(/responsibility). I am no longer a kid who gets into trouble and then asks dad and mom to cover up the mess. I am a grown-up now.

the up is a clip of my diary. Could you please proofread and tell me the responsibility is to be taken or taken on? and is it just one responsibility or a lot of responsibilities?

In a diary, I don't see that it makes much difference with those choices. 'Take on' makes more sense for 'assume new responsibility', and the plural seems more meaningful than the uncountable.
