Don't add endings (like -s or -ed) to verbs when they occur with the auxiliaries 'do', 'does', or 'did'.

"DOES IT TAKE" is correct. The suffix -es added to the operator "do" does the job of the conjugated verb (here: take) in the third-person-singular questions of the simple present tense. The same in the negative: It does not take.
