In a Psychiatrists notes what does the 2 (degree) symbol mean?

In general, it can mean "Second". That's a 2 with a small "oh" superscript, not really a degree symbol, and I believe it comes from the Latin "secundo". Note the "o" on the end of the Latin word. You'll also see that "o" with 1, for "primo" -- Latin for "first".
