could one answer by saying: "well" or "very well" or "fine" or "good"?

2) Is there a difference between

How did you spend your summer holidays?

and

How did you spend your summer holiday?

Are they both acceptable?

3) If I use vacation(s) instead of holiday(s)

is it equivalent? Which one sounds more British or more American?

No. You need to offer a description of what you did.

As your first question, you can say all of those. However, you should be careful because the response to how should be an adverb. But when you say "fine" or '"good" you are actually saying. "it was good"
As for your second question, both are acceptable, however I have heard "summer holidays" more often.
Last, vacation is the American alternative of holiday.
