Such as/as/comma

Are these sentences correct:

1-They failed to prevent such deadly accidents as in our factory.
2-They failed to prevent deadly accidents as in our factory.

3-They failed to prevent deadly accidents, such as in our factory.
4-They failed to prevent deadly accidents, as in our factory.

"1" and "2" define a particular kind of "deadly accident".

In "3" and "4", "as" and "such as" are just supposed to mean "for example".
