Success/fine/nomination

Hi!
I'm doing an Exam practice and have run into some trouble.

The Orto Botanico, in Padova, Italy, is the world's oldest, and home to a (11) ____ fine collection of rare herbs. Interestingly, since 2002, UNESCO will (12) _____ only one nomination a year from countries which already have a World Heritage site.

Which are correct? Perhaps B in no 12?

In this one I must think of one word which collocates with 'success'. Judging from the context, success should combine with an adjective to mean 'bad success', 'no success at all'. Here's another one:

No, it wasn't all the homework, it was just a few questions I couldn't find any answers to (not even in Oxford). The most difficult for me was the one with (15) gone.
