The on-line medical dictionaries suggest no special meaning for 'substitute', mdl.

It means: to serve in place of another medicine.
Example: Suppose you prescribed Dexorange syrup for iron deficiency, and this syrup is in shortage in the market. Here, you need to prescribe another syrup which serves the same purpose i.e., you shoud prescribe an iron supplement syrup with different brand name.

Brand name is Dexorange manufactured by Franco-Ind

Yes, you can. E.g. Medicine A is out of stock, so it is replaced by medicine X, which can be used for the same illness. Hence medicine B is the replacement.for medicine A.

You have to use the word "Subsitute" in case the product is in shortage.

You can use the word " Replacement" if the prescribed product's seal is broken/damaged/tampered.
