It means: to serve in place of another medicine.
Example: Suppose you prescribed Dexorange syrup for iron deficiency, and this syrup is in shortage in the market. Here, you need to prescribe another syrup which serves the same purpose i.e., you shoud prescribe an iron supplement syrup with different brand name.
