Subjuntive in reported speech

Could someone shed the light on the issue.

Do we need to switch subjunctive in the following sentences?

(It is time he made another call) - He said it was time he made/had made another call.

(She feels as if he came back.) - She said it felt as if he came/ had come back.

He said it was time he made another call.

"I feel as if he has come back."
She said she felt as if he had come back.

He said it was time he had made another call.Oof! I don't hear that sort of combination. If I did, I would say it probably had the same meaning as the version without had. The inclusion of had seems intrusive and superfluous to my ear. Maybe some people say it that way, though. You never know.
