Cambridge: "stretch something ? out to put out your hand, foot etc in order to reach something"
But this verb or phrasal verb is used in physical exercises. Is that a different verb or the "out" is added to illustrate the activity? Please explain it to me.

"Stretch out your hand" is transitive. To "stretch out" before exercise is intransitive. Same verb "stretch". You can transitively stretch out individual body parts before exercise, like stretch your legs out, and you just have to rely on context in that case. The "out" is different, though. The first one means something like "away", and the second, "completely".
