Stressed or under stress / strain ?

Hi !

Please, those people who are overweight (fat people) and that are in a health resort/health spa in order to lose weight, are they called patients or is there another word to refer to them in this case? And the professional who gives gym classes in this kind of place is called fitness instructor/ gym teacher ? Finally, when a person is too tired we say that he is stressed or under stress / under strain ?
