"No I'm not on steroids but thanks for asking"

I have seen the above as an twitter/ bbm / facebook updates..
why should on normal case such a question be asked to a person? or on the contrary does it mean the person is hiding some pains to himself and just showing of that he is strong?

Please suggest..
