My interpretation of these two word is that "State" usually is associated with negative situations or events of high level of concerns. i.e.
State of emergency.
Someone in vegetated state.
Someone in the state of shock.

A condition - is typically brief and changeable, like: The current weather in the Sierra is in "white-out" condition but expected to clear; just in time
