My interpretation of these two word is that "State" usually is associated with negative situations or events of high level of concerns. i.e.
State of emergency.
Someone in vegetated state.
Someone in the state of shock.

A condition - is typically brief and changeable, like: The current weather in the Sierra is in "white-out" condition but expected to clear; just in time for the big holiday.

The seriously injured patient from the accident was listed in critical condition last night but expected to recover after the surgery.

These are common collocations. Though, you may hear a house hunter say" the house was in really poor condition", or " the house was in the state of disrepair". I hate to tell learners this; until one has lived in a totally immersed English environment and paid enough attention to how the native use the language, there is no easy way to distinguish these usages. In the old days, when internet was just a science fiction, we wore out several dictionaries during the learning process. Now with internet, learners have a great tool at their disposal. Learning English is much easier today.
