Starting a Sentence with "He" and Looking Back Up

Okay, so I've got two different queries here:

Is there a rule about when not to start a sentence with "He"?

Second, is it sloppy to write that someone (speaking in a distressed manner) looks back up at someone else during a dialogue between them, if it wasn't written previously that the person had looked down in the first place at some point.
