Space Before and After Forward Slash "/"

A. Dear Sir/Madam
B. Dear Sir / Madam

C. SEMINARS/TRAINING COURSES ATTENDED
D. SEMINARS / TRAINING COURSES ATTENDED

1. I was wondering when to use a space before and after forward slash "/".
2. Could you please advise which is correct in each pair above?

Please note that I intend "seminars" and "training courses" as two separate items, not to be understood as "seminars courses" and "training courses".

Thank you in advance.

I use a space when the item on one side or the other is a phrase; if they are both single words, no spaces.
