Song/rap about boy having cancer

I listened to this song/rap on Facebook a long time ago and its an Australian boy singing about him finding out he's got cancer and not telling anyone then his mum finds a note or I think the rap about it and went to find him. They make him have surgery but it's too late and he dies. Thanks
