Some idioms with HOW

Hi,
three questions:

1)How do you mean? - Is this the same as "What do you mean?"

2)How so?
- Is this a way to say "Why?" or "Why is it that way?"

3)Look! How many people! - Is this wrong and should be "What a lot of people!"?

Actually, it was supposed to be separate. I meant, "Wow, how many people!" should not be ok, right? Should it be "Wow, what a lot of people!" instead?
