"Is it real?" is asking for assurance that "it" (construed as as object, substance, artifact, or the like) is not something fake, an imitation, or imaginary in some way. It is like saying "Is it genuine?"

-- Well, it's not real! It's simulated fur.

-- Did you see the diamond ring Irma is wearing?
-- Yes. Is it real? Do you know?
-- Her fiance is rich, so I suspect it's real, all right!

"Is it for real?" is asking if "it" (construed as the communication just imparted) is true. It is like saying "Really?" or "Truly?".

-- I just won the lottery! 50 million dollars!
-- Is it for real? / For real? / Really?

-- There's been an accident. Our secretary has ended up in the hospital?
-- Is it for real? / For real? / Really?

In some cases theese expresssions could be used interchangably, but I'll try to explain how they would be used differently.

I'd say "real" means "actually existing, rather than imaginary."

"For real" has more of a sense of "true, authentic, sincere."

So -- "Is he for real?" would mean "Is he really what he seems to be? Is he sincere?" Suppose someone was telling me about her new boyfriend - he is kind, he is handsome, he has fifty million dollars, he loves children, he insists on washing the dishes every night..." I might say "He sounds too good to be true! Is he for real?"
