Social support

Hi,

I need to ask a patient about his social support, but my sentence is too long. Can you say the same thing, but in shorter sentence?

Is there someone with whom you feel comfortable chatting, who will listen to you when you need to talk, whom you could really count on to help you out in a crisis situation?

But without more context, I would right it as ;

Is there anyone who you feel comfortable talking to?
