Is this correct? Is it natural to say “smell when/if”?

That is natural colloquial speech in American English. Good grammar wants an "it" in there, the same "it" that we often leave out of "I hate (it) when you do that":

"I have a hidden talent of being able to smell it when someone has a cavity."
