Sleep rough

A man named John Bird says in this video, "...sleeping rough..."

What would be the difference between "people sleeping rough on the streets" and "people sleeping on the streets"?

I haven't viewed your video, but the answer to your question is that 'rough' adds a graphic to the idea of sleeping in the streets, which we presume is rough anyway, if we think about it. Adding 'rough' means that we don't have to think so much about the conditions as we read.
