Singular verb versus plural verb

Hi all, I have a question regarding on the use of singular/plural verb.

(Question 1)
Does debbie eats chocolates everyday?
or
Does Debbie eat chocolates everyday?

Debbie = subject
Eats = singular verb?

(Question 2)
How does the teacher evaluates the students’ performance?
or
How does the teacher evaluate the students’ performance?
Teacher = subject
Evaluates = singular verb
