Should the sentence be; "Occasionally, the only information to find these professionals, is a name and address," or; "Occassionally, the only information to find these professionals, is their names and addresses". Maybe it should be written completely different? Please help.

Both are fine, although I would write "the only information available" in both cases and get rid of the second comma. Hope this helps!
