Since/ever since

Are these two sentences mean the same ?
1.I have know him ever since he was 4 years old.
2.I have known him since he was 4 years old.

Nearly. 'Ever' emphasises that I've known him for a lot of years.

I wouldn't use 'ever' if he's now only six years old.

You have to use present perfect here. It's something which refers to the present.

I have known him (ever) since he was four years old. correct
I know him (ever) since he was four years old. wrong
